These two-part, carbonless forms are preprinted with easy-to-follow instructions on how to properly perform essential home oral care. Topics include power-assisted brushing, manual brushing, manual flossing, power-assisted flossing, oral irrigation, interproximal brushing, and dental stimulator usage, among others.

The forms can be personalized easily by placing a check next to the appropriate home oral-care recommendations for each patient, along with a space to indicate how many times per day the patient should perform the care. The patient takes one copy home for reference; the other copy is filed with the patient`s permanent record.
