Stepping Stones to Success offers a booklet of forms designed to facilitate obtaining and documenting informed consent for the patient for whom Spanish is a first language. The booklet contains separate, single-page forms describing 10 commonly performed dental procedures, including impacted tooth removal, endodontic treatment, periodontal treatment, and fixed and removable prosthodontics.It also includes a general-consent form.
The forms are written in laymen`s terms on letter-size sheets, suitable for photocopying. Separate English translations are included. Obtain legal counsel prior to use.
