Every day, women worldwide look in the mirror, and where they used to see healthy hair, they now see an increasing proportion of scalp. Each elderly person is an individual with unique dental needs and values, but preservation of key teeth can have a very significant impact not only on oral function but also on their psychological well-being and general quality of life. Endodontics can play a role in this.
Traditionally, surgical implant cases have been the domain of the surgeon and the dental laboratory. However, dental professionals who use a chairside CAD/CAM restoration system now have the option to design, fabricate, and place implant crowns without using a dental lab. In doing so, the dentist becomes much more intimately involved with the nuances of the case.

What are the opportunities for lasers to treat periodontal disease in general practices? As a woman dentist, your opportunities for the use of lasers are unlimited. Periodontal disease has been a problem in dentistry since the beginning of time.
According to a study published in the Annals of Family Medicine on patient perceptions about keeping appointments, many reasons were given for missing appointments, such as forgetting, illness, trouble getting off work, child care, transportation, and cost.
The fearful adult patient often started out as a 4 or 5 year old child who was presented with dental experiences that were found to be overwhelming and less than successful, at least from the then child's perspective. However, as the hallmark of dentistry is the core value of prevention, there is special opportunity in practice to prevent current pediatric patients from developing such feelings and the resulting negative behaviors.
The objective of this treatment was to improve the health, function, and esthetics of a previously treated dentition with porcelain veneers by diagnosing and treatment planning the case as a team approach (prosthodontist and periodontist).

The best way to handle patients with past due accounts is proactively, meaning collect the uninsured portion at the time the insurance is filed so there is no reason to be billing patients except in rare circumstances. Every office should have a designated business assistant whose job title is "Financial Coordinator." The most effective way to collect past due accounts is through telephone communication, with a letter being the last resort.

The dental operating microscope provides many benefits through magnification and illumination. It is considered to be such an essential part of the endodontic armamentarium that since 1998, the American Association of Endodontists has required that "all endodontic postgraduate students demonstrate proficiency using an operating microscope." Practitioners in other areas of dentistry are choosing to add it to their practices as well.

Punctuality, presence, preparation, promotion, and professionalism are factors that will positively impact the hiring decision.
In the age of adhesive dentistry, restorative techniques are now based on retention of tooth-colored materials with conservative preparations. The goal has been to develop durable and reliable adhesion between the restoration and the tooth. Adhesives in dentistry continue to be a dynamic field of interest, particularly with the surge of esthetics.
The proposed technique eliminated the need for a custom tray, occlusal wax rim, and denture teeth wax try-in by using a previously fabricated immediate denture at the appropriate occlusal vertical dimension that satisfied the patient's functional and esthetic demands. The technique considerably reduced the number of office visits and chairtime.
Laser light can be used in the periodontal field as an adjunct to conventional therapy or as a tool in periodontal/oral surgery. Lasers are also used on oral hard tissues in restorative dentistry. In endodontics, lasers represent one of the most effective means of bacterial reduction and microorganism elimination in the root canal. Lasers can substitute some of the tools used in dental procedures presently but ideally lasers should be used as adjuncts to periodontal/dental therapy.
General dentists and specialists are familiar with both intraoral and extraoral orthodontic appliances utilized to achieve dental and skeletal movement. However, they are not used to seeing "screws" on the patients' oral mucosa. What is going on in the orthodontic world? Having the most up-to-date equipment is just another way of showing our patients that we really care. Since we use X-rays every day in our profession, why not implement a digital radiography system that is faster, easier, better, and safer than traditional film X-ray? We chose our new system as a team, armed with as much information as possible. We were trained in a few hours on a Friday, and Monday we went digital all the way. And the rest is history.
The use of lasers and laser calculus-detecting probes in my hygiene practice has shed a new "light" on the possibilities for improved patient care. Because of these technological advances, my job as a hygienist has never been more exciting and rewarding on a daily basis. For some patients, this new technology offers renewed hope for treatment they had to forego previously because of other health concerns.
Would you like to change something about your smile? When I ask that question to audiences packed with dental professionals, usually 30% to 50% (sometimes more) of the audience indicates an interest in change! So, imagine what interest lies within those who are in your existing patient family. The key is to ask! One effective tool is a smile evaluation form. Remember, great communication = great production.
In a down-trending economy, it's easy to look at aggressive solutions like cutting expenses, reducing production goals, or investing in more external marketing. But most dental patients will continue to strive for ideal care, invest in their health, and pay any fees if they like you, value their experience, are committed to a lifetime of dental care, and the office can make it affordable. Enhanced customer service creates strong relationships, so take it up a notch!
Dental implants have been a revolutionary advancement in dentistry. It is often the best treatment option for replacing missing teeth. The first step toward incorporating implant dentistry into your practice is selecting ideal patients for your first implant case. Here are some basic principles that can serve as a starting point for implant dentistry.
Dr. Kaneta Lott offers readers a valuable patient education tool in her dental sealants article. Give this to your patients to encourage them to consider sealants for their children as a protection against decay.
CT imaging, software planning, and the use of stereolithographic guides allow for successful planning, placement, and restoration of implants. As this case shows, numerous applications are possible, and guided surgery should not only be used for immediate loading of dental implants. Anatomy, bone quality and quantity, and esthetics of the final restoration were each evaluated in presurgical planning, leading to esthetically pleasing and stable full-mouth reconstruction.
Considering that the motivating factor behind most orthodontic treatment is the desire to have a more esthetic smile, that same objective can be used to help improve the oral hygiene of these patients. Once it's explained to them that carbamide peroxide and hydrogen peroxide readily penetrate the teeth and that those molecules go in all directions â?? even under the brackets â?? that initial interest turns to motivation.

We are also exploring the implant process with 3-D treatment planning and surgical implementation systems. 3-D imaging has and will continue to add a new dimension to our practice in many ways.
The lips, teeth, and gingiva are the components of the smile. The lips are the frames of the smile, and their design and volume have a great influence on the amount of gingiva and teeth exposed during rest, function, and smile. The incisal edge of the maxillary central incisor is the most important factor in the creation of a smile. It serves to determine the proper tooth proportion and gingival level.
Ankyloglossia, a developmental anomaly of the tongue, can lead to breast-feeding problems, atypical swallowing habit, speech problems, as well as preventing mouth cleaning of the buccal vestibule. The author presents two cases where infants exhibited abnormal frenums that resulted in difficult nursing. Surgical treatment with an erbium laser was utilized to incise the frenum to release the tongue.
When we interact with people whose styles compliment ours, we're immediately at ease and communication seems fluid. The challenge is people who have different social styles than ours. If we know our own social style and can objectively assess another person's style, we have the opportunity to make the other person feel more comfortable. This allows us to create an optimal experience for each patient, and also works wonders for the flow of the office.

Here's a handy checklist to help you in your quest.
Sorting through the myriad of dentin bonding systems and products on the market can be time consuming. When bonded restorations fail prematurely, it affects both the bottom line of the practice and the experience of practicing dentistry. This is exactly why there are so many different systems and so much money is invested in the development of bonding systems — to try to maximize efficiency, bond strength, and ease of technique all in one product.
Dr. Jean Furuyama offers a quick practice tip this month to optimize bonding success.
Dental clinicians often encounter small lesions on the lips and surrounding areas that raise the question: How far outside the oral cavity does the scope of dentistry extend? Recently, the Er,Cr:YSGG laser device has been granted clearance by the U.S. FDA for use in dermatological applications, which can expand its utility in a dental practice. Here are two clinical cases in which an Er,Cr:YSGG laser was used successfully to remove lesions surrounding the oral cavity.

Vulvar cancer is the fourth most common cancer of the female genital tract. In 2008, roughly 3,460 cases of vulvar cancer will be diagnosed in the United States and about 870 will die of this cancer, according to the American Cancer Society. The symptoms may or may not be obvious. But like many cancers, the earlier the cancer is detected, the more curable it is. Vulvar cancer has a high cure rate, as long as it is detected and treated early.

We must also understand that the systemic health of each patient is important to consider: Are medications that the patient takes going to influence the mouth? This two-part article will address these considerations with regard to the most commonly prescribed medications in America, and the typical medications that we as dentists use on a daily basis.

During the course of a dental procedure do you ever drop your head forward, tip to one side of your stool to access the oral cavity, or round your shoulders while leaning forward? Most dentists do, and don't even realize it. After a long day of seeing patients, are you able to painlessly and efficiently treat the last patient of the day as you did the first? If you can't, small ergonomic changes can make a big difference in your overall health and well-being.
Aromatherapy has been a popular buzzword in this world of natural healing and feeling good. Commercial and consumer interest has been driven to put "aromatherapy" on everything that smells. This trend has now spread among the dental community as a "perk" for our patients and staff in the dental office. The intent is to have that holistic "edge" to offer our patients, to be different than the other dentist down the road. We think of it as an economical practice builder.
The problem of xerostomia is more common than most people realize. Every day dentists see patients with smooth surface decay (Class V) and recurrent caries and restore the form and function beautifully. It is not enough just to restore these lesions; the etiology should be examined, and steps taken to prevent further problems. Treating the signs and symptoms of the disease (the caries) is important, but pursuing the reasons for the change to a caries-forming environment is part of the job.
There are numerous articles on how to manage the stress of juggling work, child care issues, and household responsibilities, but most of them don't get to the root of the problem. It is my observation that stress for practicing dentists originates from three fundamental issues — lack of organization, ineffective time management, and mismanagement of finances.
Thinking of your dental office environment as one of healing may seem like a new paradigm, but arguably it speaks to a core principle of your care. Patients regard your advice about their dentistry. They hear your words, experience your skill, and acknowledge your supportive staff. Then they take their cues by what they see. And they want to see an office with visual harmony that promotes the health equal to that which you deliver to your patients every day.
In part 1, in December, we looked at the drugs most commonly prescribed to our patients. In this article, we will review the medications used by dentists on a regular basis and how they interact with medications, diseases, and conditions that are commonly found with our patients. There are many good and easily accessible references for this information available. Of course, there are new medications approved every year, so make it a habit to learn about new drugs that our patients take.
Over the past two years, I have delivered a Women's Health Program across North America to female dental professionals. One of my goals was to inspire women to make simple changes to their lifestyles that would lead to maximum health benefits. Along the way, seminar attendees shared their concerns with me, including lack of sleep, stubborn body fat, overeating, bladder control, high blood pressure, and heart disease.
What first comes to mind when you think menopause? Hot flashes, night sweats, mood swings? Approximately 36 million women in the United States are in the postmenopausal phase of life. Most understand that as they age, osteoporosis can affect their bones, but sometimes the teeth and gums are forgotten. By educating our patients, we can help them prepare for this stage of their lives by creating awareness and providing advice about preventive steps that can be taken.
Osteoporosis causes more than 1.5 million fractures each year. Women are at greater risk of losing bone mass than men. Osteoporosis is often called a "silent disease" because bone loss occurs without symptoms. Although there is no cure for osteoporosis, there are treatments to prevent it or to slow or stop its progress. Risks and benefits must be considered with all treatments and age, underlying health factors along with personal preferences must be considered when comparing treatment options.
Imagine a world where diagnosing patients involves observation, tongue diagnosis, and pulse diagnosis as opposed to a dental mirror, an explorer, and radiographs. The Oriental medical model of diagnosis involves the first scenario. It's crucial that dental professionals learn more about these complementary theories and approaches. According to the American Medical Association, one-third of your patients are currently seeking acupuncture or another "alternative" therapy.
Women and men doctors have some divergent views about women and depression, according to a recent survey conducted by the Society for Women's Health Research, a Washington, D.C., based advocacy organization. Women doctors are more likely than their male counterparts to believe that women are specifically susceptible to depression during two key times of hormonal transition in a woman's life: puberty (67.5 percent to 48.2 percent) and perimenopause (92.8 percent to 67.5 percent).
Diabetes can take a toll on a person's overall health, but amidst the possible problems that may arise sexual dysfunction is not often talked about and is more common than most people realize. According to the Joslin Diabetes Center in Boston, roughly 35 percent of women with diabetes may experience some form of sexual dysfunction related to their disease. Although the number of people suffering from diabetes is at an all-time high, there are measures people can take to cut their risk.
Introduced in the United States in 2001, the Cone Beam CT (CBCT) is another modality that is becoming more popular within the dental community for TMJ disorder evaluation. It not only allows a shorter scanning time, but, depending on the machine, uses six to 10 times less radiation to patients than the medical CT and, moreover, at a lower cost.
Infertility affects one in six couples today. This staggering statistic is comprised of 40% women issues and 40% male factor with the remainder considered unexplained. Although infertility is not a woman-only syndrome, it is often the woman who endures most of the stressful impact both emotionally and physically. These jarring statistics raise many concerns and questions. When should a woman seek treatment?
You think of dental tools and your handpiece comes to mind. Or your laser. Or maybe your Cavitron. It's time to consider adding hypnosis to that list. Hypnosis is an invaluable tool for dealing with your patients' pain, fear, and other issues like bruxism, TMJ, and unwanted habits such as smoking, nail biting, teeth grinding, that arise in your office. You may even choose to use hypnosis as a personal resource to help you lower your stress levels so you can be more productive.
Are you exhausted? Do you have pain all over but can't figure out what's wrong? If so, you may be suffering from fibromyalgia, a chronic condition that causes exhaustion, sleep disturbances, and diffuse pain in your muscles, tendons, and ligaments. Fibromyalgia is more common in women than in men, but the reasons remain unclear. Even today, it is estimated to take an average of five years for a fibromyalgia patient to get an accurate diagnosis.
A variety of terms are used and confused when describing the problems that our patients have with "dry mouth." The term xerostomia is a person's subjective self-perception of having a dry mouth. However, xerostomia is also often incorrectly and globally used to describe a reduction in salivary flow. When a person has a change in the quality and/or quantity of saliva, he or she has salivary dysfunction or salivary gland hypofunction (SGH).
It is likely that each of us will have to deal with the problems of insufficient saliva, whether it leads to patients with recurrent decay or with discomfort from a removable prosthesis. But the future looks very interesting for saliva to have another valuable use: the diagnosis of systemic diseases by analyzing the components of a patient's saliva. The UCLA Human Salivary Proteome Project, or HSPP, has found more than 1,000 distinct proteins in saliva.
Two areas in which the pediatric dentistry community sees great potential in caring for children's teeth and addressing related systemic health issues include utilizing saliva to diagnose health and disease states and using dental stem cells for dental and facial repair and regeneration. While saliva for clinical utilization is still a few years away, studies have shown that almost anything that can be measured in blood can be measured in saliva.
The National Osteoporosis Foundation recently announced results of a nationwide survey that reveals a concerning low awareness of bone health and risk for osteoporosis in American men and women age 45 and older, as well as a lack of conversations regarding bone health between patients in this age group and their health professionals. In fact, a woman's risk of a hip fracture is equal to her combined risk of breast, uterine, and ovarian cancer.
Many people haven't ever heard of celiac disease, but for the millions of people unable to eat bread, cookies, pizza crust, and pasta, it's a reality they have to live with every day. Celiac disease is an autoimmune digestive disorder that wreaks havoc on the body's intestines when foods containing gluten are consumed. It affects roughly twice as many women as men. In the United States, it affects two million people, or about one in 133 people.
With an estimated 14 million cases worldwide, you would think more would be known about scleroderma, a chronic, debilitating disease that can lead to the hardening and tightening of skin and connective tissue. But it seems that we don't know enough; more research is needed. What we do know is that scleroderma is at least three times more common in women than in men, according to the National Institutes of Health. The cause remains unknown.
Over the past few decades, the incidence of melanoma, the deadliest form of skin cancer, has increased. Melanoma has become the most common cancer in women between the ages of 25 and 29. Certain people may be at higher risk for developing skin cancer. Melanoma is very treatable if caught early; that's why it is so important to be aware of the signs and to visit a dermatologist regularly. Here are some ways you can protect yourself from the dangers of sun exposure.
A teleconference on June 10 featured leading experts on the health dangers of mercury amalgam dental fillings, as well as an attorney who represents Consumers for Dental Choice, a nonprofit consumer advocacy group. The teleconference focused on the recent dramatic reversal of the U.S. FDA's position on dental fillings containing mercury. On June 4, 2008, the FDA's Web site publicly acknowledged their potential neurotoxic effects ...
There are approximately 400,000 people with multiple sclerosis (MS) in the United States today. Although the cause is unknown, MS is at least two to three times more common in women than men. Because of improved methods to limit disability, the life expectancy of individuals with MS is now nearly the same as that of unaffected people. Patients can have full, productive lives, while working with their health-care providers to manage their disease.
Less than 10% of Americans have participated in a medical research study of any type, according to a recent survey by the Society for Women's Health Research. Thanks to the Internet, there are now opportunities to participate in research online. That's good news for patients who want to volunteer for research, but can't because of work, family, and other time-consuming commitments.
What was once speculation is now being confirmed by scientists: the brains of women and men are different in more ways than one. Scientists are looking into ways that sex-based brain variations affect the thought processes and behavior of men and women differently. Discoveries could point the way to sex-specific therapies for treatment of various neurological conditions, which, in turn, will mean better health outcomes for all patients.
Despite the fact that folic acid can prevent some of the most common birth defects, young women aren't getting enough of it. Only one in three women ages 18 to 24 takes a daily supplement containing folic acid, according to statistics from the Centers for Disease Control and Prevention in Atlanta, Ga. This age group accounts for roughly 30% of all births in the United States. Here are a few tips for women to make sure they get all the folic acid they need.
Oral aspects of child neglect and abuse are well known to the dental health-care team. Craniofacial injuries occur in more than 50% of the cases. Studies show that abuse occurs at all socioeconomic levels and when it comes to damage, there is no real difference between physical, sexual, and/or emotional abuse. All that distinguishes is the abuser's choice of weapons. Health-care providers must identify children at risk, educate families, and report suspected cases of abuse.
Over 65% of North Americans are overweight or obese, and diseases related to inactivity and overeating are on the rise. Even more alarming is the increasing number of overweight children. After decades of research and experience, we have identified some simple action steps that will have a significant impact on the health of your family. Next time you're in your doctor or dentist's office, take a moment to look around the reception room. Is the carpet clean? How about the receptionist's desk? Next, visit the restroom. Is the sink clean and shiny? How is the soap dispensed? First impressions influence our opinions, and it's easy to see how a messy, unkempt waiting room or dirty bathroom would make you question the infection-control practices used in the dental treatment areas.
You see that magical blue line and a torrent of emotions rushes through you. In the next nine months, your body will go through amazing changes as it prepares for the marathon of birth. You imagine yourself huge as a whale, with little energy and many aches and pains. But wait! These are images of pregnancy past. Modern-day research has shown that exercise during pregnancy can have numerous benefits.
Oncologists, endocrinologists, and hematologists are all concerned about the possibility of osteonecrosis as a very rare complication of bisphosphonate therapy, but dentists should be more concerned. The risk of the complication of osteonecrosis of the jaw needs to be weighed against the oral rehabilitation of the patient. We should not act alone regarding bisphosphonate therapy. As always, thorough medical histories and physician consultations are imperative.
The holiday season is upon us — truly the best of times, yet it can also be the worst of times when it comes to personal health and wellness. By taking a few minutes to follow these simple suggestions, you'll come out of the holiday season feeling more refreshed than you went in.
If you ask several people you know, many would say they do not wake feeling rested and are fatigued during the day. It is likely that this same group would say that they snore or wake several times during the night. Sleep disordered breathing as sleep apnea or snoring is extremely prevalent yet often undiagnosed or underdiagnosed. Dental professionals with the proper training can have a huge positive impact on these patients' overall health.
The end of the year is upon us. After all the shopping, eating, and family time, we have a few days to catch our breath before the start of the New Year. We are reminded to change our fire detector batteries when the time changes twice a year, but are we ever reminded when to look into our medicine cabinets to see what drugs have expired? While you're at it, take a look at the dates on dental materials at the office too.
Research published in the American Academy of Pediatrics official journal, Pediatrics, reveals that there are an estimated 500 pediatric deaths a year associated with children's cold and cough medicine. The American Academy of Pediatrics and other child advocate groups have issued warnings to physicians and parents about the dangers associated with using OTC cough and cold medication for children under 6 years of age.
Whoever coined the phrase "my job is a pain in the neck" could have been a female dental professional. Compared to the average female worker, female dental professionals experience two to four times more musculoskeletal pain. They also report higher frequencies of neck and shoulder pain than their male counterparts. Poor posture, movement, or imbalances in the neck or shoulder can result in numerous neck/shoulder pain syndromes, including cervical instability.
A new University of Colorado at Boulder study indicates that not only do human hands harbor far higher numbers of bacteria species than previously believed, women have a significantly greater diversity of microbes on their palms than men.
If you're thinking about New Year's resolutions, consider making 2009 the year you try eating the Mediterranean way. It's not only delicious but also appears to protect against heart disease and many other chronic conditions. The January 2009 issue of Harvard Women's Health Watch suggests some ways to get started eating Mediterranean-style.

But somewhere in the back of your mind will be the ghosts of resolutions past — the ones that never saw the light of Valentine's Day. How can you ensure your resolutions have a chance of becoming a reality? Follow this prescription ...

In the United States, millions fight a life-and-death battle with eating disorders. More often than not, dentists can provide the gateway to critical medical treatment for these individuals. By becoming aware of certain signs and symptoms, you can uncover illness that may otherwise go unnoticed — and untreated. With the right knowledge, you can save a life.
Mind and body are really two halves of the same whole. Each profoundly influences the other. Depression and heart disease are a good example of this duality. People who are depressed are more likely to develop heart disease, and those who have heart disease are more likely to fall into depression. A regular exercise program can improve mood even as it strengthens the heart ...
Some dentists are using screening devices that they say may help identify cancers and premalignant lesions. But experts are divided on whether these tests will reduce mortality from oral cancer or simply lead to a wave of expensive and unnecessary biopsies.
This is the personal story of Dr. Melissa Connell, a pediatric dentist in Chicago, who became a kidney donor for her brother in 2004. In Dr. Connell's words: "I'm glad I chose to be a kidney donor. It is my greatest accomplishment thus far. I hope to inspire others to do the same if they're ever faced with the choice. I gave Eric the gift of life, and in return I feel an overwhelming sense of joy and fulfillment that can't be put into words."
In part two of this series, we will discuss the Exposure Control Plan (ECP) in more detail, pointing what you need to do to be in compliance with OSHA requirements. The Bloodborne Pathogens Standard prescribes certain precautions that must be followed whenever there is the reasonable expectation for a worker to be exposed to blood/OPIM.

What can you do to create and maintain a productive, healthy team? There are numerous elements of building a great team — vision, goals, accountability — but one of the most often overlooked or inadequately done is establishing position responsibilities. Here's how you do it.
If you like to project yourself as a no frills, hard-hitting lady (like I do), you might back away from some of the softer elements of leadership. The last thing you want is to appear vulnerable and emotional with your team. It can be difficult for a woman in a leadership position to show sensitivity for fear it will be perceived as weakness. I now believe you can be a black-and-white, hard-hitting leader who creates and communicates through vision and values.
The author details five ways for a woman to look and act sensational.
If you find it difficult to juggle home, office, and outside interests, and to make matters worse, you keep adding more onto your plate, how can you simplify your life? Dr. Houseman has some ideas.
Have you considered working at a community health center? In July of 1998, I accepted a position at Westside Health Services, a federally qualified CHC in the city of Rochester. While working at the center, I was exposed to a diverse and underserved population with a variety of dental needs. In October of 2001, I was offered a position as a dental director. My experiences are positive and have given a new dimension and purpose to my life.
Perfectionism is tough for dentists. We demand perfection in our life and our work, strive for it, struggle with it, and then are devastated when we are unable to deliver it. Many dentists place unreasonable demands on themselves and then feel "less than" when they fall short. Extreme perfectionism robs you of peace of mind and satisfaction with life. It is exhausting because so much energy is spent on being perfect. Here are some tips to help you rein in your fixation on perfection.
So, Dr. Jane Smith, it has been a month since we busted the leadership myth and have placed the burden of leading your team directly on your shoulders. It's easy to say I can't do this; I want to be my team's friend, or I will leave it to my office manager or husband to lead. But the buck always stops with the dentist as leader, and it is your softer skills that will make or break your success.
For many dentists, discussions with a business partner take place strictly in the office. For others, business matters might be discussed in the office, at the breakfast table, or in the car on the way home from work. That is the daily routine for dentists whose business partners are also their spouses. In this article, meet three dentist couples who have their own ways of working things out.
In 1997, Richard Carlson, PhD, published a little book that became a big phenomenon — "Don't Sweat the Small Stuff ... and It's All Small Stuff." I argue with Carlson's premise. We do sweat the small stuff, because it's the small stuff that finally makes us crazy. My challenge for you is to figure out what's behind the "small stuff" and how to leverage the situation to your best advantage.
One of the hardest things to do as leader is to recognize the problems of your practice when it comes to staff, systems, and cash flow issues and not take those challenges personally so that you can be proactive instead of reactive. There are so many elements to a small business that a woman dentist can easily lose her perspective. Luckily, the business world has already done the research and come up with a model to diagnose organizational challenges.
I find that one of the greatest obstacles blocking our path to success is the attention we pay to those voices in our head. You've heard them â?? the ones that tell us we're not good enough, that we'll never have the practice of our dreams, that we'll never find the man of our dreams ... the list is endless. We struggle with those voices day after day. Here's a step-by-step process to help you quiet those voices and replace them with voices that will carry you to success.
Ever think about traveling to another country to provide dental care? I first thought about it after an interview for dental school, but not again until last fall when the opportunity arose to go on a weeklong trip to Jamaica in the summer. Even though we provided a great service to numerous people, I wish we had been able to provide more complete care. We performed mostly extractions, and one of the most important tools we lacked was the ability to take radiographs.
As a woman dentist and practice leader, you have the power to create a culture of positive growth. How? By delivering feedback that helps your team learn, continuously improve, and become more self-directed through a collaborative growth conference. In these formal coaching sessions, both dentist and team member share their thoughts on the individual's strengths and challenges, define personal goals, and co-create an action plan for continuous improvement.
Procrastination. It happens to most of us one time or another. Procrastination leads to stress, deadline crises, loss of productivity, disapproval of others, and feelings of guilt, laziness, or even stupidity. It zaps your energy. Procrastination is so much more than time management skills. You could have all the time in the world to just get it done, and yet, you still procrastinate. Why? There are many reasons, but six tips can help get you into action.
Heart disease, the No. 1 killer of women, may hit closer to home than many realize. The American Heart Association's Go Red For Women movement recently released a study revealing the nation's 10 most — and least — heart friendly cities for women. Minneapolis-St. Paul took the title of Most Heart Friendly City For Women with the lowest cardiac mortality rate for women, low hypertension rates, and highest exercise rates.
For the past 11 years I've walked three times a week for one hour with Judy, usually starting at 6 a.m. This routine of 60 minutes of conversation while moving the legs is great for the body, and imperative for the mind and soul. These conversations refine our thought process, make us laugh, keep us sane, and help us find solutions! The conversations YOU have with your team, patients, and loved ones can also be healthy catalysts for growth and resolution.
How well are you in control of your choices? You can't save time in a bottle and pour it out to use another day, nor can you turn those 24 hours into 30. The good news is that you are the one person who has control over your time, and you gain this control by your choices. Are your choices leading you to the life you want or taking you down a path of disenchantment? The choices you make will determine the places you'll go, and the choices you make will determine the life you'll live.
On March 6, Dr. Roy S. Shelburne of Pennington Gap, Va., was convicted of 10 felony charges involving racketeering, health care fraud, structuring funds, and money laundering. He was convicted by a federal jury in Abington, Va. According to the prosecution, he maximized his practice's income by overbilling insurance carriers and Medicaid for services that were never performed and for services that were performed, but not medically necessary.
Hindsight provides many answers to the question, "If I knew then what I know now, what would I have done differently?" When I graduated from dental school in 1976, there were no business courses, no courses on how to get along with people, nothing on practice vision, and certainly no insights into balancing life and dentistry. Dental school was all about teeth. I've learned most of what I know now through the school of hard knocks. Here's my list of what I've learned.
An article written about the May 12 Sichuan earthquake disaster in the Shanghai Daily discussed the outrage and grief of the families affected by this calamity. Fortunately you don't have such disasters in your dental practices. Yet, you do encounter difficult situations, whether it is presenting a scary diagnosis, working with a challenging patient, or handling an awkward moment at the front desk. Here's how you can communicate the hard stuff in your practice.
Dentistry and sports merge in the exciting practice of sports dentistry — a composite of skills for treatment, prevention, education, and research. Sports dentists who devote time to professional, collegiate, and Olympic teams treat only a small percentage of the athletes in this country. Statistics on athletic oral and dental injuries support the large number of youth and adult athletes in need of sports dentistry's expertise. Here's a brief look at areas of special interest.
Being a good boss is paramount to keeping exceptional employees and maintaining a viable dental practice. What makes a great boss? Here are 15 qualities. Most anyone can be a boss. Women see the world differently than men. We interpret events differently; we value different things. We have a different life experience. Why has this difference been seen for so long as inferior? Until now, the "norm" has been based on the male view of the world. And the female view has been the "different" view. But today, the male condition is not the "norm." We are not the aberration from the "norm." We are our own "norm."
Many of us find change difficult so we avoid it at all costs. We tell ourselves we like our life just the way it is, and that becomes our justification for inaction. Why rock the boat? Yet, life gets stale and we yearn for growth and newness. What's a person to do? In order to embrace change, you must change the way you look at change. Change is an opportunity to create something new so that your life becomes more fulfilling. How wonderful is that?
How often do you receive a standard envelope in the mail? How many times have you heard "I'm away from my desk or on the phone. Please leave a message at the sound of the tone, and I'll get back to you as soon as possible"? How frequently do you get asked, "I didn't catch your name. What's your name?" Take off your sign that reads "I am so predictable." Get creative!
We consider the most important factor of an ideal continuing-care department to be the purposeful conversations that happen during the adult prophy, periodontal maintenance visit, or the quadrant of scaling and root planing. These conversations identify and address patients' motivators and concerns; educate about oral health issues; build commitment to a lifetime of oral care; support self-esteem and success; and encourage referrals.

Many things come to mind when I think of patient compliance. One is the importance of patient compliance in order for dental treatment to last and be totally beneficial to the patient. Another is the importance of the dentist and hygienist to be able to work alongside one another to achieve patient compliance. Other things that come to mind are the patients. There are many parts to this puzzle, but we can be very influential in helping patients be compliant.
It used to be that when a patient had a good experience in the dental office, the highest compliment would be to tell someone about it — another parent in the carpool, maybe, or a neighbor she ran into at the grocery store. Now it seems that the first person to hear about your patient's experience is a complete stranger who logs on to one of many Web sites that invite users to review everything from dog groomers to plastic surgeons.

Are you tired of routine, trite gifts, such as ties for men and logo-clad calendars and mugs for customers? Some standard gifts may not project the excitement you're looking for this year as you prepare to give gifts to the important people in your life. You want to do something more relevant and creative when it comes to gift giving for your family, friends, referral sources, team, and patients. Follow your heart, and gift giving can become a joy for both the giver and receiver.
Overhead: One word, three syllables, endless frustration. The thought of it can suck the joy right out of your best day. So many great ideas, so many things you could pursue, so many improvements that could be made if only you had the money. But it always seems that no matter how hard you work, more and more of the practice revenues go to cover the day-to-day costs of doing business. There's hope. Read on ...
"A New Year's resolution is something that goes in one year and out the other." The author is anonymous, but it could have been said by any one of us. How many New Year's resolutions did you make? Reality check: With a few weeks in, how are you doing? Are you pleased or frustrated? Are you ready to toss out the whole darn resolution thing? Go ahead! Toss the resolutions and make this year different. Successful people set goals, not resolutions. Furthermore, they actually achieve those goals.

There has never been a time when professionalism, care, and attention to detail have been more important. In these changing economic times, it is imperative that you focus on doing your best, and that applies to each person on the team. Each person can make or break a relationship with a patient. Review your systems and ask yourself, "How can we do everything a little bit better?" It's the little things that make a big difference.
Different people express love and friendship in different ways. You must recognize which "love language" you speak and which the other person speaks. Once you do this, your relationships will deepen and become more satisfying. That's the power of the languages of love.
Emotional/social intelligence (EQ) is now considered to be a powerful predictor of important life outcomes, but it's tough to define. Simply stated, it's the mental ability to reason about emotions and to use emotions to enhance thoughts. In tough economic times, leaders and individual team members can be taught to manage emotions for the good of the practice. A dental practice can realize improved business results and morale as a result of EQ coaching.
Conflict happens. At one time or another we have all fallen victim to it. The experts refer to conflict as "interpersonal disputes." Call it anything you want, but no matter where you encounter conflict, it comes with feelings of discomfort. I can retreat to my shower or car and safely argue and nobody argues back. I always win! But here's the rub. Is it about winning, or is it about finding a reasonable solution?
The first step in becoming more positive is to place a moratorium on the news. Stop listening! Stop watching! Stop talking about it! Remember that you have control over what you allow into your mind. Declare your space as a no negativity zone! Create a positive attitude zone. Here are some team habits you can incorporate into your day ...
During these tough economic times, many of us are just happy that we're still in business. But it's important not to forget the sound business strategies that have supported your practice in the past. And the very first step that anchors any yearly business plan is a well-thought-out fee increase that matches your vision, values, and goals. Here are four tenets of fee setting you can follow so that you will never have to approach another increase with trepidation.
One of the most important elements for creating an outstanding dental practice is leadership. Without it a practice will lack a strong culture, motivation, and dedication, and this will ultimately affect the bottom line of the business. The greatness in people can be brought out if a leader gets creative. Leadership comes in different forms, styles, and actions. Here are some leadership tips for being whom you need to be to attract and keep the right team.

Columnist Carol Tekavec, CDA, RDH, advises us to keep in mind that reimbursements for out-of-network dentists may or may not be included in best practices requirements for some insurance carriers.
Columnist Anne M. Oldenburg says that after receiving a minor disciplinary action, a dentist should inform her insurance provider and possibly retain legal counsel at the initial investigation stage.
The exit strategy needs to be developed far in advance of the time you decide to retire.
Dental benefits for inlays, onlays, and crown build-ups are unpredictable.
Many dental offices still use different ID numbers on paper and electronic dental claims. Some use Tax Identification Numbers (TIN) or Social Security numbers. The NPI allows dental practices the ability to have one identification number for everything.
In most dental offices, no one thinks about technology. Most decisions are made by the doctor, and are emotional or necessary decisions. What should be done is a systematic approach to initiating, integrating and upgrading office technology. Establish a technology road map with rest areas to guide your office's technological decisions.
How are your dental fees determined? Do you allow insurance companies to dictate the fees you charge or the type of dentistry your patients receive? Our patients and their dental care are uppermost in our minds. We must be very careful in not allowing the insurance companies to influence our decisions for patient care. Always remain INSURANCE AWARE â?? NOT INSURANCE DRIVEN!
Imagine not working with a mortgage broker and not paying hidden mortgage broker fees. It's now getting easier, and more convenient to get a home mortgage over the Internet.
Identity theft is one of the biggest personal safety issues of our time. Identity theft, in addition to consumer fraud, affects millions of unsuspecting people each year. They are having their privacy invaded in a personal manner. According to the Identity Theft Protection Information and Resource Center, every 79 seconds someone becomes a victim of identity theft.
It's never too early to think about forming an exit strategy for your dental practice. In fact, having a good exit strategy should be an important part of your business plan. Make sure your dental practice is marketable when the time comes by concentrating on making it thrive now â?? and making sure it continues to thrive until you're ready to sell.
Do you feel at times like your practice overhead is out of control? Do you feel like your entire team is working hard to have adequate practice production, and yet you don't know where the money is going? Does it seem like you're working so hard to have so little? It's easy to take control and keep control in just a couple of hours a year. With an annual business plan in place, you'll never be under the spell of the Wicked Witch again!
While more and more dental plans are covering implants, annual plan maximums make the patient primarily responsible. This article focuses on discussing implants and insurance with patients. Here are some examples of common questions and easy-to-understand answers. It is important for patients to understand the basics of how implants work, as well as the limited nature of dental insurance coverage.
At some point in your career, you may need to evaluate your current practice location and make the difficult decision of continuing to rent space or buying your own facility. For some, the decision will come quickly and easily. For most, the decision can be quite difficult and many factors must be considered. Take time up front to assemble a team of professionals who will help you achieve your goals and help you avoid the "money pit."
Despite a good patient population, good risk management, and — most important — good patient care, media attention raises public awareness to dissatisfied patients who suffer an untoward result, a complication, or who just did not pay their bills. Articles that do not portray the dental profession in a favorable light can spark a dissatisfied patient to seek legal advice. Practicing good risk management must continue to remain a priority in your practice.
Increased utilization review has put a spotlight on radiographs. The need for documentation of the reasons radiographs are taken is now essential for many plans to pay claims. The ADA "Guidelines for Prescribing Dental Radiographs," outlines current thinking on radiographs. While the final judgment as to which X-rays are necessary remains with the dentist, the "Guidelines" provide a powerful argument for insurance carriers looking to limit expenses.
Issues relating to independent contractors in your office setting and referrals to specialists for potentially serious conditions often involve risk concerns. Recognition of these concerns is critical. Having good office procedures, protocols, and systems in place, emphasizing good documentation, and assuring that those who practice in your office are responsible, qualified practitioners who follow your office procedures are essential to minimize your exposure.
Working with dental insurance can be a difficult prospect. Many times patients don't really understand their benefit plans and expect that their insurance will cover every service that they need. When faced with the prospect of paying a portion of the fee, many will tell their dentist, "Just do what my insurance pays for." Here are some tips to help make working with dental insurance a little less stressful.
Hiring is the most important thing you do, once you have your vision. However, dentists often become frustrated with the interviewing and hiring process. Personnel issues aren't at the forefront of most dental school curricula, so many dentists feel like they're taking a stab in the dark when it comes to managing these systems. Here are tried and true techniques to help you proceed with confidence, focus, and direction, leading your new hires to do the same!
Designated professions are mandated by law to report maltreatment of children. Dentists are usually in this group of mandatory reporters. Some state laws also include dental hygienists or assistants. The circumstances under which a mandatory reporter must make a report vary by state, but generally one must be made when the reporter in his/her official capacity suspects or has reasonable cause to believe a child known to him/her has been abused or neglected.
Practical Claims and Coding column: "Informed Consent for New American Heart Association Guidelines"
Drastic changes in American Heart Association guidelines for the prevention of infective endocarditis were based on findings of a writing group which determined that about 90% of people who were previously considered to be at risk are no longer of concern. For decades, antibiotic premedication was recommended to reduce the chance of spreading infection from mouth bacteria released during dental treatment. New evidence shows this may not be necessary.
You already have so much on your plate, the last thing you want to think about is long-term-care insurance. Caregivers may view LTC insurance as just another expense in already over-stretched budgets, but it could end up protecting savings in the long run. It's worthwhile to at least check out the benefits of LTC for your parents or even for yourself. Here are some shopping tips for long-term care insurance.
We may try to trivialize the importance of the telephone, but it's clear in the world of dentistry that it is the primary "point of entry" for new patients. Even as technology advances, Alexander Graham Bell's invention remains the first impression that can make or break your success in getting prospective new patients to schedule. Having a friendly voice and enthusiasm is imperative to convert callers into patients of record.
It is not uncommon for a dentist to receive a request for a refund for a payment sent in error by an insurance carrier. The payment may have been processed incorrectly; paid for a person who is no longer eligible for benefits or who is no longer an employee of a certain company; or for any number of other reasons. Is a dentist required to refund the money? What rights does a dentist have when faced with this request? Let's take a look at answers to these and other questions.
Certainly, in the current economy, everyone is much more aware of how far the dollar isn't going these days. However, many dentists fail to recognize that they are among the few professionals with far more control over their financial situation than most. The first step in controlling your own practice economy is having some understanding of your numbers and how your office compares to the following overhead benchmarks.
From the Broker — Whether you plan to purchase a practice or look for an associate position, identifying the best opportunity can be daunting. Here are some key things to zero in on when interviewing with a practice owner. From the Financial Planner — Depending on which career path you take into the dental community, your responsible actions will follow you and help you create opportunities. Here are some financial steps you need to take now.
There are certain aspects of record keeping that are basic to the endeavor, whether a dentist is using paper or computerized charts. According to a mandate presented in 2004 by President Bush, health-care providers are expected to be using digitized records by 2014. That is only six years away! Here are a few of the hurdles currently facing digitized records.
Each year, millions of Americans turn to the Yellow Pages to locate a dentist — for themselves, their children, and families. Consumers turn to the Yellow Pages because it is a trusted resource where they can quickly see a landscape of providers in their area. As consumer shopping habits change, Yellow Pages products are evolving to meet their needs. Here, we've outlined the top 10 most frequently asked questions from dentists.
The words "I'm sorry," when expressed with sincerity, can soothe angry patients and reduce the likelihood of lawsuits. Oftentimes it is a poor bedside manner that drives the angry patient to pursue a lawsuit after a bad result. Addressing a patient's concerns promptly, providing proper and sufficient information about current and future care, and understanding the patient's needs and fears goes a long way toward minimizing the risk of getting sued.
Business ownership is risky and stressful, but taking that risk can have enormous long-term rewards. To do well in private practice, dentists must be salespeople who can sell themselves and the services they offer. They've got to be teachers who can teach dental health to often unwilling patients. Most of all, they've got to have guts! There are some differences in how women manage the pressures of motherhood and business.
Many professional women experience complex webs of business and personal relationships on a daily basis, composed of balancing career and family. With full plates personally and professionally, the pressure of chasing down overdue patient debt is an added stress, which derails many women dentists from focusing on other aspects of their practices. The sooner you find help from a third-party collection agency, the better your chances of recovering overdue debt.
The ADA coding system undergoes updating and revision every two years. Under an ADA licensing and royalty agreement, I also publish a single coding handbook with all codes, guidance for claims, information on narratives and attachments, and other information whenever the codes are revised. For 2009-2010, there are only four new codes with 29 description wording revisions, so only an addendum is being added to my handbook.

Implant dentistry is a proven treatment option for patients with missing teeth. While the number of implants placed has increased, so have patients' questions about whether or not an implant is for them. Further complicating the issue is the fact that many dental insurance plans do not cover implants. Here are some specifics.
The greatest obstacle to success is complacency. The Economic Bailout Bill is going to affect you in some small ways but it is not intended to bail you out from your financial problems. You cannot rely on government to help you increase your net worth. You need to take charge of your own financial decisions! There is hope for dentists! Some of the most successful practices I counsel continue to produce at the highest levels they have in the past three years.
From the Broker — Associates, beware! You may be missing out on extra income. From the Financial Planner — Be aware of the cost of lost opportunity in remaining an associate for too long. Be sure your economic picture is clear.
A written and detailed treatment and fee estimate is an essential ingredient for the successful care of patients. Patients need to understand what treatment is being recommended and why. This goes a long way toward reducing misunderstandings, and ultimately assisting collections and reducing the potential of malpractice lawsuits. Having all aspects of treatment and fees discussed in advance enhances an atmosphere of trust between all parties.
In these tough economic times, it's more important than ever to be proactive and take effective action against the poor economic forecasts. An integral part of any successful money management program is promotion and advertising. A good rule of thumb in a down economy is to increase promotion, and one of the best ways to do that is through the use of the Internet.
No profession is viewed as a necessary evil as much as accounting. Accountants are constantly telling people what to do and what not to do with their money. By changing their view of accountants, dentists can use their knowledge and experience to improve their practices. Ultimately, accountants want dentists to spend more time doing what they love — dentistry — and less time doing what they probably don't ...
Every time you hire someone to join your team, you strengthen or weaken the practice infrastructure. If the composition isn't right, if the fit isn't correct, or if they simply aren't prepared to withstand the pressure, you compromise your team, your production, and your success. And since when were you willing to compromise? Next time you're hiring, focus not on filling the position but on finding the right person for your practice.
I cannot emphasize enough the importance of proper documentation. It should tell a story about the patient's dental visit. It is one's "key witness" in a court of law, and one's best defense as well. As a dental colleague of mine who is an expert witness in disciplinary hearings and malpractice suits has told me many times, "It really doesn't matter until it matters." In this article, the author gives tips for proper documentation for both paper and electronic record keeping.
One of the greatest challenges for dentists is the "how to run the business" side of their practices. This is especially true for marketing — you need marketing to build a practice that allows you to provide your services, yet for most, the distaste of doing it combined with the need for consistency of execution means most dentists get only minimal results. Luckily, one PR strategy can help achieve optimum results — expert positioning.
The use of fluorides in dentistry has a long history. Professionally applied topical fluoride continues to be one of the most successful and easy-to-use procedures for the prevention of caries as well as the treatment of hypersensitivity of teeth for both children and adults. The ADA has given us three codes to use when reporting fluoride treatments to third parties.
If you're like most people, you're trying to stay positive as you watch the stock market slip. You may be seeing fewer new patients, or existing patients may be delaying treatment. When it's essential to find a way to make ends meet every month, it's key for your practice to offer what others simply aren't. Here are some tips that can make an immediate, positive impact on your practice even as the current financial turmoil presents challenges to your success.
Dentists associating with owner-dentists has become more prevalent in recent years. However, as associate dental relationships continue to flourish, so do unfortunate and unnecessary legal disputes related to these associate relationships. The following list highlights points that should be addressed in any correctly drafted associate dentist agreement. Addressing these issues will help prevent later legal disputes.
From all outward appearances, things seem to be just fine. Most of your patients come back. But look closer over time. Is your patient base shrinking a bit in reality? Often, patients are lost at precisely the point where the patient experience begins — the telephone. In many cases, simply educating staff on effective telephone communication can prevent the loss of hundreds of patients and tens of thousands of dollars every year.
Judging by my family's frequency at the dental office over the years, I've put a couple of my dentists' kids through college. My bright white smile was bought. I am a dentist's dream client. Patients who keep coming back, like me, know a few things about a well-run dental office. And I know when something is not right. In fact, that's why I fired my first dentist. Here's my story, and here are some tips to keep your best clients coming back.
In most cases, an unemancipated minor's consent to treatment is not valid. A parent or legal guardian's consent must be obtained before treatment is rendered. Unfortunately, many dentists still provide "routine" dental treatment to minor patients without obtaining informed consent from a parent or legal guardian.

Very few people aspire to spend their life as a professional speaker. I had no idea, until age 35 that this could be my next 30-year career. Holding monthly team meetings, educating patients during case presentations, or being an officer in your local, state, and national dental associations all require a confident, organized, and totally prepared presentation. Competency creates confidence!
Twenty-five years ago, I graduated from dental school. During the course of these past 25 years, what I have learned is: life happens. Either you stop and enjoy it, or it flows on around you. The next two years will be especially difficult if you were hoping to sell your home, refinance your debts, or borrow money.
