Add efficiency through forms

Are you covering all of the bases? Have you questioned all your recare patients thoroughly? In today's busy practices, it's easy to fall short on a thorough interview that covers both dental and medical issues, as well as all updates on personal demographics and insurance information. Is one staff member in each department supposed to obtain the information that pertains to that department, or do you just hope that everything will be covered somehow? The solution is actually just a form away.

A recare update form will give you confidence that all issues have been addressed. The form provides an efficient way for you to obtain all that you will need for a complete patient interview.

This form will become a tool that helps you:

• determine your patients' dental needs and wants
• evaluate your practice through the eyes of your patients
• have the patient commit to a payment method for that appointment
• update patient records
