ProSystems, in collaboration with the National Society of Dental Practitioners, offers five miscellaneous dental forms that will help reduce your exposure to malpractice loss. The forms were designed by Burton R. Pollack, DDS, attorney-at-law, one of the nation`s foremost dental-risk managers.
The forms include charge-slip transmittal forms; reminder notes; medical-alert stickers; X-ray, medication, prescription and referral-tracking log; and emergency telephone-communication forms.
